Someone takes a photograph from slightly above. Under bright sunlight, the small patch you assumed was an ordinary hair whorl suddenly looks much larger.

The crown is easy to miss because it sits outside your normal view. By the time thinning becomes obvious in photographs, the process may have been moving quietly for years.

That does not mean the area is beyond help. Crown hair loss often responds reasonably well to treatment—particularly while fine hairs are still growing there.

Is it balding or just a hair whorl?

Everyone has a whorl at the crown. Hair changes direction around this point, allowing some scalp to show even when density is normal. Short hair, wet hair and overhead lighting can make the centre look surprisingly bare.

The useful clue is change.

Has the visible circle become wider over the past year? Does the scalp show in ordinary indoor light now, rather than only beneath a bathroom spotlight? Are the hairs inside the area finer and shorter than those around the sides of the head?

Take a photograph in the same room, with dry hair, once a month. Six nearly identical pictures tell you much more than examining the crown every morning and trying to remember how it looked yesterday.

If the area is gradually expanding and the hairs are becoming finer, male-pattern hair loss is the likely explanation.

What is happening beneath the scalp?

Crown balding is usually caused by androgenetic alopecia. Genetically susceptible follicles react to dihydrotestosterone, better known as DHT.

The hair does not simply fall out and disappear overnight. Each new growth cycle produces a slightly thinner, shorter strand. A sturdy hair eventually becomes soft and barely visible. Dermatologists call this miniaturization.

The follicles around the sides and back of the head are usually less sensitive to DHT, which creates the familiar horseshoe pattern as hair loss advances.

Genes can come from either side of the family. Looking only at your mother’s father is not a reliable prediction. A thick-haired father does not provide immunity either.

Crown loss may appear on its own or develop alongside a receding hairline. The two areas do not always progress at the same speed.

Minoxidil is often the first move

Topical minoxidil has a long track record for thinning at the crown. It can extend the growth phase of hair and help miniaturized follicles produce thicker strands.

There is no dramatic change after two weeks. Early on, some users notice extra shedding as hairs switch cycles. Visible improvement commonly takes several months, and six months is a more sensible checkpoint than six weeks.

Application has to reach the scalp, not merely coat the hair. Foam is usually neater and may irritate less, while liquid can be easier to spread through longer hair. Our comparison of minoxidil foam and liquid explains the practical differences.

Minoxidil works only while it is being used. Stop, and the hair maintained by treatment will gradually be lost.

Using extra does not speed up regrowth. It mostly wastes the product and increases the chance of itching, flaking or unwanted facial hair. Rogaine is the best-known minoxidil brand, though regulated generic versions use the same active ingredient.

Finasteride tackles the DHT side

Finasteride takes a different approach. It reduces the conversion of testosterone into DHT, slowing the hormonal process that shrinks susceptible follicles.

For many men, the main success is not dramatic regrowth. It is keeping the crown from becoming noticeably worse. That can feel underwhelming after three months and extremely valuable after five years.

Finasteride is FDA-approved for male-pattern hair loss in men. Results take time, and continued use is needed to preserve the benefit.

Sexual side effects—including reduced libido, erectile difficulty and changes in ejaculation—can occur. Most men do not report them, but they should not be waved away. Mood changes and fertility plans also belong in the discussion before treatment.

Propecia is the original hair-loss brand of finasteride. Women who are or may become pregnant should not handle crushed or broken tablets because exposure can harm a developing male fetus.

Some dermatologists prescribe low-dose oral minoxidil or topical finasteride off-label. These options may suit particular patients but need a clinician’s input. Oral minoxidil can affect blood pressure, heart rate and fluid retention.

What about shampoo and supplements?

A shampoo can make hair look fuller and keep the scalp comfortable. It cannot usually reverse genetic crown balding.

Biotin is useful for genuine biotin deficiency, which is uncommon. In a man eating a reasonably varied diet, another high-dose hair vitamin is unlikely to rescue DHT-sensitive follicles. Large biotin doses can also distort certain blood-test results.

Iron, thyroid or nutritional testing may be appropriate when shedding is sudden or widespread. Classic slow crown thinning usually has a genetic explanation, not a missing vitamin.

When the pattern does not look typical

A smooth round patch that appeared quickly may be alopecia areata rather than male-pattern loss. Scaling and broken hairs can suggest a fungal infection. Pain, burning, redness, crusting or shiny scar-like skin needs a dermatologist.

Sudden shedding from the entire scalp may follow illness, surgery, rapid weight loss or severe stress. That is another problem entirely and needs another plan.

For ordinary crown balding, early treatment offers the best odds. A thinning crown still contains living follicles. A crown that has been smooth and shiny for years leaves medication much less to work with.

The cruel thing about crown loss is that other people often see it before you do. The useful thing is that a photograph can catch it while there is still time to act.

Have you noticed that your hair is becoming thinner and duller?
